Please complete and submit this document accompanied by your payment (cash or check) to the Cashier's office either in person (MAT 354N), via the payment drop box located down the hallway to the right of the Cashier office window or by mail to:

You may also submit your request and remit payment (by web check or debit/credit card) on-line via MyUW.

There is a 3-5 business day processing period and students who opt to pick their transcripts up will do so at the Enrollment Services office (GWP 102).
