CAP Application

In order to receive assistance, the Childcare Application and supporting documents must be completed and submitted to the Department of Student Services. Once approved, assistance is available for all 3 quarters (excluding summer) within one academic year.

Please print and complete all forms (note that some forms will need to be completed by your childcare provider) and submit to the Department of Student Services. If you have any questions please contact Jennifer Magofna.
