Study-Abroad Enrollment Form

This form is required of all students participating in a UWT study-abroad program; students will not be registered until the completed, signed form is received in the International Programs office. Additional paperwork may also be required for specific study-abroad programs.

Budget Request Form

This form will assist you in working with Financial Aid to cover the costs of your study-abroad program. Also visit the website for the study-abroad program you are participating in, to find the budget information specific to your program.

Academic Planning Form for Study Abroad

For students participating in non-UWT programs, this paperwork will assist with planning for placement of credits earned while studying abroad. In order for students to earn UWT credit, this form and the study-abroad enrollment form (see above) must be submitted to the International Programs office before departure.

Evaluation Form

This form enables students to provide feedback after their study-abroad experience is complete.
